Payroll Deduction Pledges will renew automatically each successive year. Existing pledge payments can be changed at any time by resubmitting the form below. To stop payroll deductions, contact Lauri Stark.

Instructions:
1. Select your pledge amount below selecting the checkbox in the corresponding Pledge Selection field. If you'd like to pledge a different amount, select "other" and input your pledge on the line below.
2. Complete the required employee information and preferences, sign, then click Submit Your Pledge at the bottom of the form.

Note: Payroll Deduction Pledges can begin at any time during the year using the form below. The minimum contribution per pay period is $5.00. There is no maximum. An employee’s decision to participate or not participate in this voluntary program will have no effect on the terms and conditions of employment. All amounts are confidential. Employee giving names will be recognized in the Foundation's Annual Impact Report.
